Insurance

Our practice understands that patients rely on their dental insurance plan to help defray the costs of dental services. If you have dental insurance, we will gladly file your claims on your behalf and accept payment for the portion covered, but in order to do so, we need your understanding and cooperation regarding the following insurance policies:

  • While the filing of dental claims is a courtesy that we extend, fees charged are the responsibility of the patients from the date services are rendered, regardless of insurance coverage, and accounts will age accordingly.

  • After each visit please be prepared to pay your estimated co-payment.

  • The treatment we render is based on your particular dental needs. However, insurance companies do select certain services that they will not cover. We encourage you to become familiar with your policy exclusions, limitations, deductibles, and required co-pays. WE CANNOT GUARANTEE BENEFITS.

  • Since most insurance companies pay on a percentage basis, we would like to point out that the insurance company decides on what portion of our fee they are going to base their payment on; hence the amount they have determined within their company to a fee schedule, which bears no relationship to the current standard of cost in this area.

  • Because any changes with your insurance, such as name, address or coverage will affect our ability to file a claim and delay payment, it is important that you notify our front office of these changes upon your arrival.

  • If there is no response from your insurance company, we will make every effort to collect payment by resubmitting the claim. However, in most cases after 60 days we will have done all we can to collect. At that time payment should be made to our office and you may wish to contact your insurance company for reimbursement. Please understand that the insurance policy belongs to YOU and we have no leverage to obtain payment from your insurance company.

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.

When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable time frame. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
